If you have tested and confirmed positive for covid-19. Please see and fill out survey below. The following survey should be completed by patients with hidradenitis suppurativa (HS) who have experienced COVID-19 infection, or their caregivers. If you are completing this survey on behalf of an HS patient, some questions may not be relevant or answerable.
